Guidelines for addendums

If a provider calls to review a case:

If the original reading radiologist is available, defer the call to that rad (best option). If the original reading radiologist is NOT available, you take the call AND:

  1. You believe an addendum is needed but is NON URGENT, you can contact reading rad to addend at their convenience via email or phone OR you can addend. If you make the addendum, flag the case for peer review and/or let the original reader know. Substantive addendums to the original report should include the provider that was contacted and date/time of notification. OR,
  2. You believe an addendum is needed and the change is URGENT or would affect patient care, you should addend the study and flag the case for peer review. You can contact the original reader to let them know about the case. Addendum MUST include the provider that was contacted and date/time of notification as with critical results.
